Appointments
Appointments may be made Monday-Friday from 8:00 a.m. to 5:00 p.m. Please call 919-851-5482 to schedule an appointment. All patients are seen by appointment only. We always try to arrange a visit with the physician of your choice. If that is not possible, you will be offered the opportunity to see our physician assistant, who will be familiar with your condition from the information in your medical chart in our office. If you must cancel an office visit appointment, please notify our office as soon as possible or at least 24 hours in advance.
For your convenience and timeliness, we request that you bring the following when you come for your office visit:
- Your insurance card(s).
- Your co-pay amount that will be requested at check-in time (we accept cash, debit cards and all major credit cards).
- Your social security number or another form of identification.
- A list of your medications and allergies (along with your pharmacy name, address and phone number).
- The patient information form and medical history form. This is information that your Doctor will need in order to treat you properly.
- All relevant medical records, lab results, x-rays from your Primary Care Doctor (without this information our Doctor will not be able to evaluate you properly so we will have to reschedule your appointment). Men 40 + years should be sure to bring PSA lab results from their primary care doctor.
- Your Primary Care Doctor's name, address and phone number.
- Authorization number from your Primary Care Doctor's office faxed prior to your visit (if your insurance company requires an authorization- see list of insurance on our home page.) Without the required authorization, you may be asked to sign a waiver and pay for your visit or reschedule your appointment.
For more information or to schedule your appointment call 919-851-5482.
