Emergencies
If you believe you have a critical medical emergency, please go to the nearest emergency room or call 911. For urgent urology problems, you can reach the on call physician by dialing 919.554.8539.
Patient Forms - Urology Care
Please download and complete the Medical Health History form, the Patient Registration form, and the Financial Policy statement and bring the completed forms with you to your appointment. These patient forms may also be completed online and then emailed directly to the AUNC Office you will be visiting.

If the registration forms are filled out in advance, we ask you to arrive fifteen minutes before your appointment time, otherwise please arrive thirty minutes before your appointment to allow sufficient time to register and complete the necessary paperwork. Remember to bring any X-rays, MRIs or other diagnostic information relevant to your problem.
