Appointments
To schedule an appointment please call us at (919) 782- 1255. A receptionist is available at our office from 8:30 a.m. until 5:00 p.m. Monday through Friday, except holidays. For problems requiring same-day attention, please call the office as early in the morning as possible.
Please notify us at least 24 hour in advance if you are unable to keep an appointment. This will enable another patient to use the appointment time, a courtesy you may appreciate yourself in the future. We will reschedule your appointment for a time convenient for you. We reserve the right to charge for appointments that are missed.
Please bring all the medications you are currently taking or a complete list of the medications (including the dose of medicine and how often you take it) to all appointments. The medications you are taking provide important information to your doctor about your overall health and other medical conditions that you have. Also, certain medications need to be stopped or held temporarily before some procedures and it is important for us to know if you are taking any of these.
Be sure to request refills at the time of your appointment with us. We are also happy to call in refills on most medications, please have the pharmacy telephone number available when you call us. If you have not been seen by one of our providers in some time we may request that you be evaluated before calling in any new medication. Some medications, particularly narcotic based medicine for the treatment of severe pain, cannot be called in and may require an evaluation with one of our providers before being refilled. Be sure to notify our office and the pharmacy of any known drug allergies you may have.
If you believe you have a critical medical emergency, please go to the nearest emergency room or call 911. For urgent urology problems, please call our main office number (919) 782-1255. Calls made after hours should be reserved for urgent matters only.

Please download and complete the Medical Health History form, the Patient Registration form, and the Financial Policy statement and bring the completed forms with you to your appointment. These patient forms may also be completed online and then emailed directly to the AUNC Office you will be visiting.

Appointment Request Form
Initial Appointment
If the registration forms are filled out in advance, we ask you to arrive fifteen minutes before your appointment time, otherwise please arrive thirty minutes before your appointment to allow sufficient time to register and complete the necessary paperwork. Remember to bring any X-rays, MRIs or other diagnostic information relevant to your problem.
Other Information:
All patients, please bring insurance card with you. If you were referred by your primary physician, have him/her fax us a referral letter and any other pertinent information. Please let us know if your insurance company requires treatment at a specific hospital or if it uses a specific laboratory for any testing that might be done in our office.
Insurance and Payments

Just like any business our practice requires you to understand your financial obligations. Please take a moment to download, read, and sign our Patient Information Form which states our financial policy. We require all patients seen in the office to sign the financial policy prior to being seen by our physicians.
