We pledge to treat our patients with the highest quality, state-of-the-art urological care. We will always treat our patients in a respectful, courteous, and professional manner.
Central Call Center
All incoming calls are managed by our Central Call Center, which can be reached by calling (919) 758-8677. Please organize your thoughts prior to calling our office. Our professional Call Center personnel will assist you by answering your question or route your call to your Physicianís Care Team. Your doctorís Care Team consists of a Care Coordinator and a Nurse/Medical Assistant. The Care Team provides individualized and personalized care between you and your physician. They will assist you in scheduling, changing or cancelling medical office appointments, outside referrals, surgeries and imaging studies. If you are calling to make an appointment, please have your insurance information readily available. Your Care Team will also help you by answering questions about your condition, treatment and obtaining pharmacy refills.
If you have an urgent Urological problem, please inform our Central Call Center personnel.
Calls made after hours should be reserved for urgent matters only. If you believe you have a critical medical emergency, please go to the nearest Emergency Room or call 911.
Please call (919) 758-8677 to schedule an appointment. Please have your insurance information readily available when you call. Our friendly call center staff is ready to help you Monday-Friday from 7:30 a.m. to 5:00 pm. All patients are seen by appointment only. We always try to arrange a visit with the physician of your choice. If that is not possible, you will be offered the opportunity to see one of our other physicians or one of our midlevel providers. If you are an existing patient, our providers will be familiar with your condition from the information in your electronic medical records.
If you must cancel an office visit appointment, please notify our office as soon as possible, or at least 24 hours in advance, otherwise you may be charged a no-show fee.
We request that you bring the following when you come for your office visit:
Your insurance card(s).
Your co-pay amount, which will be requested at the time of check-in. We accept cash, debit cards and all major credit cards.
Your social security number or another form of identification.
The name, address, and phone number of your pharmacy.
Please bring all the medications you are currently taking or a complete list of the medications (including the dose of medicine and how often you take it) and a list of any allergies to all appointments. Some medications need to be stopped or temporarily held prior to certain procedures so it is important for us to know if you are taking any of these medications.
Please inform our staff if there are changes since your last office visit in your medical condition, medications, allergies, home or mailing address, or health insurance.
The patient information form and medical history form if you are a new patient or havenít been seen in one of our offices within the past three years. This is information that your provider will need in order to properly treat you.
All relevant medical records from your primary or referring health care provider, including laboratory and x-ray results. If you have had an imaging study of your urinary tract, please contact the radiology file room personnel where your study was performed and request a CD of your study and then bring it to your appointment. Without this information our providers will not be able to evaluate you properly so it may be necessary to reschedule your appointment. Men forty years or older should bring their PSA lab results from their primary care doctor.
The name, address, phone number and fax number of your primary care provider (and referring healthcare provider if different than your primary care provider).
Authorization number from your primary care provider's office faxed prior to your visit if your insurance company requires an authorization. Click here for a list of the insurance companies with whom AUNC has a contract agreement. Without the required authorization, you may be asked to sign a waiver and pay for your visit or reschedule your appointment.
Please inform our staff if your insurance plan requires utilization of a particular hospital, lab or radiology clinic, or if your insurance plan requires pre-certification prior to a procedure of surgery. If you do not know, please contact the member services department. The appropriate phone number is listed on the back of your insurance card.

You can complete the form(s) on your computer. If you are using Adobe Reader 9 or later, you will not be able to save the typed data into the form. You will need to print out the completed form and bring the completed form to your appointment. Your preferred email platform will automatically pull up a new email addressed to firstname.lastname@example.org. If you wish to email the Completed patient registration forms at a later time, the completed patient Registration forms can be emailed to email@example.com.

Appointment Request Form
If the registration forms are filled out in advance, we ask you to arrive fifteen minutes before your appointment time, otherwise please arrive thirty minutes before your appointment to allow sufficient time to register and complete the necessary paperwork. Remember to bring any X-rays, MRIs or other diagnostic information relevant to your problem.
Laboratory Tests and X-ray Results
In order to prevent interruption to patient care, please do not contact our office for lab or x-ray results prior to your appointment. One of our practitioners will review the results with you at time of your appointment. If labs or x-ray tests were performed on the day of or following your office visit, you will be called with the results within a week or so after the study was performed. If we haven't been able to get in touch with you within a week of your test, please notify our office. As a general rule, our staff reserves time after we've finished seeing patients to return phone calls.
Please alert our staff of any allergies, and have the number of your pharmacy available each time you need a prescription. If you require renewal of prescription medication, please ask your pharmacist to fax a renewal of prescription medication request form to our office at 919-758-8723. Be sure to request refills at the time of your appointment with us. If you have not been seen by one of our providers within 12 months, we will request that you be evaluated before calling in any new medication. Some medications, particularly certain types of narcotic medicine for the treatment of severe pain, cannot be called in and may require an evaluation with one of our providers before being refilled.
