Send Records To AUNC
If you wish to request medical records from another medical office or medical facility to be sent to an AUNC Provider, click on Obtain Records From Another Medical Office to download a PDF file. Print the form, fill it out, and forward it to the appropriate medical office or medical facility. 
Send Records From AUNC
If you wish to request medical records from your AUNC provider’s office to be sent to another medical practice or medical facility, click on Send Records To Another Medical Office to download a PDF file. Print the form, fill it out, and forward it to appropriate AUNC practice location.
As a courtesy, there is no charge for forwarding your medical records to another medical office or medical facility. There is a $35 charge if the medical records are copied for personal use. Unless other arrangements have been made with our medical records clerk, only a patient or his or her legal guardian can pick up the medical records. A photo ID will be required. Requests will be processed within 5 to 7 business days. Patients picking up their records will be notified when the records are available.
